
We are currently accepting new clients. To schedule an appointment, we require a $68 Reservation Fee that will be applied to your pets exam fee at their first visit. This deposit is Non-Refundable if you do not show for your appointment or cancel within less than one business day.
Your Pet’s First Visit to Associated Veterinary Clinic

We are so excited to meet you and your pet(s)! Please help us expedite your first appointment with us by taking the time to fill out our New Client/Patient form. If you do not have access to your pet’s previous records, please contact your previous veterinary clinic and have them email your pet’s records to info@associatedvetclinic.com or they can fax them to us at 920-748-5220. You can also bring in or drop off any previous records you may have during business hours, prior to your appointment. Having your pet’s medical records prior to your appointment will help familiarize us with your pet’s medical history as well as determine if your pet is due for any preventative care prior to your arrival. All new patients will receive a complete physical examination on their first visit. This allows us to familiarize ourselves with your pet and to establish a baseline for future care.
Things to remember for your appointment:
- Please bring your pet’s medical records (if they haven’t been sent to us already)
- Please bring in a fresh stool sample (from within 24 hours)
- All dogs MUST be on a leash or contained in a carrier for safety reasons.
- All cats should be contained in a carrier for safety reasons.
Payment Options
Payment is due when services are rendered which means that it is expected that you pay for the medical care that we provided your pet at the end of your appointment. In order to focus on our patients' needs, customer service, and minimizing costs, we do not charge to accounts or provide payment plans.
- We accept debit cards, credit cards, cash, and checks.
- We also accept Care Credit. For services totaling $200 or more, payments can be made interest free as long as the balance is paid before the promotional period ends which can be 6, 12 or 18 months depending on the total and your credit score.
